Baptism Application form (from birth to age 3 years) (google form) - please fill in this online form to apply for a Baptism at St Francis Xavier, Montmorency (12 noon Saturday) or Our Lady Help of Christians, Eltham (12 noon Sunday). Baptisms are generally held on the first and third weekends of each month.
